  • Forms and medical mileage reimbursement information

San Luis Obispo Workers’ Compensation Appeals Board

The San Luis Obispo district office provides judicial services and has an Information and Assistance Unit for injured workers who need help navigating the workers’ compensation system. The statewide DWC Information Services Center can be reached at 1-800-736-7401.

Medical Mileage Reimbursement

Injured workers may be entitled to reimbursement for reasonable travel expenses related to medical treatment, including mileage, parking, and tolls. Current forms and rates are available through the Division of Workers’ Compensation.

Return-to-Work Supplement Program

Some injured workers who receive a Supplemental Job Displacement Benefit voucher may also qualify for a one-time payment through California’s Return-to-Work Supplement Program. Applications are submitted online through the Department of Industrial Relations.

Subsequent Injuries Benefits Trust Fund

The Subsequent Injuries Benefits Trust Fund may provide additional benefits to certain workers who had a pre-existing disability or impairment before a later work injury. These claims involve detailed medical and legal requirements and are evaluated separately from the underlying workers’ compensation case.

California State Disability Insurance

The Employment Development Department provides information about Disability Insurance and online claim filing through SDI Online. Disability Insurance generally applies to eligible workers who cannot work because of a non-work-related illness, injury, or pregnancy.
